Enrolling
New Providers & Clients

When you enroll a new provider/Client you are required to create an Family Friends & Neighboors (FFN) Services Intake as well as a Health Related Social Needs Screening (HRSN) Screening. This will ensure that your Providers/Clients are eligible for all the benefits and services that Sisters in Common provides. Use of the FFN Intake form is applied only to those Clients who are also Providers and are caring for one or more children. An individual who is not caring for children can be enrolled as a client only using the HRSN Screening form. Use the buttons below to access both these pages on Sisters in Common’s website. IMPORTANT!: If you are enrolling a Provider please use the FFN Providers full ID number 153XXX as the identifier in the HRSN Screening form.


Reviewing Client
Intakes & HRSN Snapshots

After you have completed a FFN Provider Intake and a Health Related Social Needs Screening (HRSN) Screening you can review your entries by going to the links below. The first is the Total Intake Listings page where all Intakes are listed starting by the intakes that were completed recently. The Second is the “Retrieve Your Screening Results” page where you can retrieve your Clients/Providers HRSN Snapshot, that represents the issues and challages that your Clients/Providers are currently experiencing. This Snapshot now becomes your Clients/Providers SERVICE PLAN which can now be used to identify the specific services or refferals that will help your Clients/Providers with their current needs!


Documenting
FFN Provider Counsultations

After you have done your Intake and HRSN Screening then you must keep a record of all contact and communications you have had with your Providers / Clients using the FFN Consultation form.This should include the areas that were talked about and notes detailing what progess was made towards solving these issues and what goals were attained. Use of the FFN Consultation form is applied only to those Clients who are also Providers that are caring for one or more children.



DASHBOARD

You are required as a Community Health Worker or a Community Support Worker to document all communications, interactions and activity that you perform on behave of Sisters in Common Clients / Providers. This Dashboard is designed as a landing page where you can access all the tools necessary to complete your documentations. Remember if an activity is not documented then it never happened, and Sister in Common cannot get money to pay for it!

Once you have completed a Health Related Social Needs Screening (HRSN) Screening with your Clients / Providers , You must keep a record of all contact and communications you have had with your Clients / Providers concerning the issues that were discovered during their HRSN Screening. This should include the areas that were talked about and notes detailing what progess was made towards solving these issues and what goals were attained.

Community Alerts & Resources

Community Alerts are used to send alerts about the most important and urgent information and resources affecting your community. These alerts are usually so urgent that they texted directly to your phone, so please pay attention to them and share them as soon as possible with your Clients / Providers because many of the alerts are time sensitive. You can use the links below to view the very latest alerts or search for older ones. You are also required to document which Clients / Providers you have sent alerts to each time you send them out. Use the Communiy Alerts Consultation form link below to complete this activity. You can select Multipule Clients / Providers in this form.

COVID Social Risk Assessments

The COVID Social Risk Assessments screening tool allows any individual to create a snapshot that will help them to assess their risk of catching Coronavirus (Covid-19) during the last 24hrs based on their social habits and interaction with other people during that time. The First link is where the Client / Provider can do an initial COVID Social Risk Assessments screening. Once the first snapshot is created it can be retrived and multipule screenings can be done that are tied to the same Client / Provider from that snapshot page.
